Job summary

Gentiva is seeking a dedicated Hospice Registered Nurse to lead in-home care for patients in Alexander City, Alabama. As a key member of the hospice team, you will provide skilled nursing, assessments, and family support in the patient’s home or other living environments.

This role emphasizes collaboration with physicians, social workers, chaplains, and CNAs to deliver compassionate, patient-centered end-of-life care with a focus on education and bereavement support for families.

Qualifications

• Minimum 1 year of RN experience in hospice, acute care, or home health.
• Hospice or palliative care experience preferred.
• Current Alabama nursing license and CPR certification required.

Responsibilities

• Be the primary contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home, assisted living, or other patient homes.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with an interdisciplinary team to develop and refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones, plus caregiver education.
