Job summary

Gentiva is seeking a Registered Nurse (RN) Clinical Liaison in Alexander City, Alabama. This ongoing staff role focuses on guiding admissions, coordinating referrals, and building strong relationships with hospitals, case managers, and physicians to ensure timely hospice care.

You will educate patients and families about hospice services, review medical records, coordinate intake, and support discharge planners while upholding compassion, compliance, and quality care across our nationwide

Qualifications

• Licensed RN with hospice experience (3–5 years) required.
• BSN with 5+ years hospice experience preferred.
• Current RN license in the state of employment.
• CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals.
• Assess eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families.
• Educate patients and families on what hospice care includes, how it works, and why it matters.
• Complete necessary documentation and coordinate intake/admission, ensuring legal, clinical, and payer requirements are met.
• Serve as a liaison between referral sources and our hospice care team with clear communication and rapid response.
• Make daily visits to hospitals and facilities, maintaining visibility with discharge planners and case managers.
• Build relationships with referral partners and identify new outreach opportunities to increase awareness.
