Angels Care Hospice in Arizona is seeking an RN Case Manager to coordinate all aspects of a patient’s home care. You will work with other medical disciplines to deliver professional care and perform the admission visit to determine eligibility for home care services.

The role involves developing the plan of care to be followed, ensuring quality, compassionate service and adherence to regulatory requirements.

Qualifikationen

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One year of experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.
