Job summary

Angels Care Hospice is seeking an experienced RN Case Manager to coordinate comprehensive home health care for patients. You will assess needs, collaborate with a multidisciplinary team, and ensure smooth admission visits and eligibility determination from the patient’s home.

Qualifications

• Graduate of an accredited diploma, associate or baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care.
• Perform admission visit to the patient in the home and determine eligibility for home care services.
• Develop the plan of care to be followed.

Education

Graduate of an accredited nursing program (Diploma/AS/Baccalaureate)
