Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate comprehensive home care for patients in Sedona, AZ. You will perform admissions, assess eligibility for home care, and develop individualized plans of care in collaboration with the care team.

The role requires AZ RN licensure, CPR, and reliable transportation. Ideal candidates have 1 year of RN experience; 2+ years preferred, with opportunities for professional growth and comprehensive benefits.

Qualifications

• Graduate from accredited nursing program (Diploma/ASN/BSN).
• Current Arizona RN license and valid driver's license.
• CPR certification and Hepatitis consent/declination on file.
• 1 year RN experience; 2+ years preferred.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care as RN Case Manager.
• Interact with other medical disciplines as needed to provide care.
• Perform admission visit, determine eligibility for home care, and develop the plan of care.
