Job summary

Angels Care Hospice in Jerome, AZ is seeking a Registered Nurse Case Manager to coordinate all aspects of the patient’s home care. You will interact with other medical disciplines as needed and provide professional nursing care, including performing the admission visit and determining eligibility for home care services.

Qualifications include graduation from an accredited nursing program (diploma/ASN/BSN), an active RN license, a valid driver’s license, CPR and hepatitis consent/declination, and

Qualifications

• Graduate of accredited diploma/ASN/BSN nursing program.
• Current RN license in the state.
• Current state driver's license.
• 1 year RN experience (2 preferred).
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient's home care.
• Interact with other medical disciplines as needed and provide professional nursing care.
• Perform the admission visit to the patient in the home and develop the plan of care.

Education
