Job summary

Angels Care Hospice in Arizona seeks a Registered Nurse Case Manager to coordinate all aspects of a patient’s home care, collaborating with medical disciplines and delivering professional nursing care at the patient’s home.

You will conduct admission visits, determine eligibility for home care services, and develop the plan of care to be followed, ensuring quality and compassionate care for patients and families.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care.
• Perform admission visit, determine eligibility for home care services and develop the plan of care.
