Job summary

Angels Care Hospice is recruiting an Registered Nurse Case Manager to coordinate all aspects of home health care for patients in their homes. You will conduct admissions, assess eligibility for home care services, and develop personalized care plans with input from multidisciplinary teams.

The role requires graduation from an accredited nursing program, an active RN license, and a valid driver's license, with at least one year of RN experience; CPR/Hepatitis documentation and reliable

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• The RN Case Manager coordinates all aspects of the patient’s home care.
• You will interact with other medical disciplines as needed & provide professional care to the patient.
• You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.
