Job summary

Compassus in Sedona, AZ, is seeking a part-time Hospice Registered Nurse (RN) Case Manager for 24 hours per week. Provide compassionate home-based care, coordinate care with the interdisciplinary team, and support patients and families in their homes.

You'll use your nursing expertise to manage patient plans, alleviate symptoms, and educate caregivers while enjoying flexible scheduling and a supportive, family-focused culture.

Qualifications

• Current AZ RN license required.
• Minimum two years as a registered nurse; hospice experience preferred.
• CPR and BCLS certification current.

Responsibilities

• Provide compassionate hospice care in patients' homes.
• Collaborate with the care team to develop care plans and manage symptoms.
• Educate caregivers and families on treatment goals and medications.
• Coordinate services across the interdisciplinary team.
