Job summary

Hospice of New Jersey is seeking a Full Time Hospice RN Case Manager (RNCM) to join our nursing team providing end-of-life care throughout the community. You will deliver hands-on hospice nursing, coordinate services, and support patients and families across Morris, Essex, Union, Passaic, Bergen and Hudson counties.

The role requires an RN license, at least 1 year of acute care experience (hospice a plus), EMR proficiency, CPR if required, and reliable transportation.

Qualifications

Hold current unencumbered license as a Registered Nurse.
Minimum one year experience as an RN in a medical surgical/acute care setting; hospice exp a plus.
Must be computer proficient in typing and EMR usage.
Current CPR certification if required by state.
Driver's license, vehicle insurance, and dependable transportation.

Responsibilities

Deliver hands-on hospice nursing care and coordinate services.
Guide patients and families through every step of the hospice journey.
Collaborate with the interdisciplinary care team to ensure compassionate, high-quality care.
Educate and advocate for patients and families to participate in care plans.
Participate in on-call duties as part of the team.
