BAYADA Home Health Care is hiring a full-time Registered Nurse (RN) Case Manager, Hospice for Secaucus, New Jersey. The role involves delivering high-quality hospice care in clients’ homes, with competitive hourly pay and weekly pay benefits.

Responsibilities include home visits, medication administration, wound care, and coordinating with the multidisciplinary team. Travel to cases is required, with a focus on compassionate, patient-centered care and support for families.

Qualifications

• Current RN license in NJ.
• Minimum of one year of clinical (medical/surgical) experience.
• Prior home care and/or hospice experience strongly preferred, but not required.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medication, perform wound care and treatments, and provide comfort care.
• Monitor client conditions and report changes to the Clinical or Client Services Manager.
• Follow up on and document doctors' orders.
• Perform client assessments as needed.
