Job summary

BAYADA Home Health Care in Ocean City, New Jersey, is seeking a Registered Nurse (RN) Case Manager, Hospice, for a full-time role. You will provide compassionate, client-centered home-based hospice care, visiting patients in designated counties. The ideal candidate holds an NJ RN license, has at least one year of clinical experience, and can travel to cases.

Qualifications

• NJ RN license is required.
• At least 1 year of recent, verifiable clinical experience.
• Home health or hospice experience is preferred but not required.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform administration of medication, wound care, treatments and comfort care.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Follow up with, execute, and document doctors' orders.
• Perform client assessments as necessary.
• Occasional on-call after hours and/or weekends.
