BAYADA Home Health Care seeks a Registered Nurse (RN) Case Manager for Hospice in Harrison, New Jersey. This full-time role involves delivering compassionate 1:1 in-home hospice care, coordinating with a multidisciplinary team, and ensuring adherence to care plans across Hudson and Bergen counties.

The ideal candidate holds an NJ RN license with at least 1 year of clinical experience; hospice/home care background is preferred. The role emphasizes travel and community-based patient support.

Qualifications

• Active NJ RN license
• Minimum 1 year of recent clinical experience
• Hospice or home care experience preferred
• Strong computer skills and EMR experience
• Ability to travel to cases as assigned

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medications, provide wound care, treatments and comfort care.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Follow up on and document doctors' orders accurately.
• Perform client assessments as needed.
• Occasional after-hours and weekend on-call duties
