Job summary

BAYADA Home Health Care is seeking an experienced RN for hospice case management in Hoboken, NJ. This Full Time role involves visiting clients at home, coordinating care with a multi-disciplinary team, and ensuring comfort and safety in the home setting.

The RN will administer medications, monitor patients, and document orders, with travel across Hudson and Bergen counties. NJ licensure required; hospice/home health experience is preferred but not mandatory.

Qualifications

• Current RN license in New Jersey.
• Minimum of one year of recent clinical experience.
• Prior home care or hospice experience is preferred but not required.
• Solid computer skills; EMR experience preferred.
• Ability to travel to assigned cases.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments, and comfort care.
• Monitor client conditions and report changes to the Clinical or Client Services Manager.
• Follow up on and document doctors' orders.
• Perform client assessments as needed.
• Occasional on-call after hours and weekends.
