Job summary
BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for a nursing job in Longport, New Jersey. This Full Time position involves visiting hospice clients, coordinating care, and working as part of a multidisciplinary team.
The RN must hold an active NJ license with at least 1 year of clinical experience; home care/hospice experience strongly preferred. Travel to cases within Atlantic and Cape May counties is required.
Qualifications
- Must hold an active NJ RN license.
- Minimum 1 year of recent clinical (medical/surgical) experience.
- Home care or hospice experience preferred.
- Solid computer skills; EMR experience preferred.
- Ability to travel to assigned cases.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Administer medications, wound care, treatments, and comfort care as assigned.
- Monitor clients’ conditions and report changes to the Clinical/Client Services Manager.
- Follow up with, execute, and document doctors’ orders.
- Perform client assessments as needed.
- Occasional on-call after hours and/or weekends.
