Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager for Hospice in Union City, New Jersey. The full-time RN will work from the Hospice office in North Bergen, serving Hudson and Bergen counties on a per-visit basis.

Responsibilities include home visits, administering medications, wound care, monitoring patient conditions, and properly documenting orders. A NJ RN license and 1 year of clinical experience are required; bilingual Spanish is a plus; travel is required;

Qualifications

• A NJ RN license is required.
• At least 1 year of clinical (medical/surgical) experience.
• Home health or hospice experience strongly preferred.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits in designated geographic territories.
• Administer medications, wound care, treatments and comfort care.
• Monitor clients’ conditions and report changes to Clinical/Client Services Manager.
• Follow up with and document doctors’ orders.
• Perform client assessments as necessary.
• Occasional on-call after hours and/or weekends.
