Job summary

BAYADA Home Health Care seeks a compassionate Full Time Registered Nurse (RN) Case Manager, Hospice to join our team serving North Bergen/Hudson and Bergen counties. The RN will make home visits, administer medications and provide comfort care, while monitoring client conditions and documenting orders.

Travel to cases is required; on-call may be needed. Qualified candidates have an NJ RN license, at least one year of clinical experience, EMR skills, and the ability to travel.

Qualifications

• A current license as a Registered Nurse (RN) in NJ.
• Minimum of one year of recent, verifiable clinical experience.
• Home care and/or hospice experience preferred.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments and comfort care.
• Monitor clients' conditions and report changes to supervisor.
• Follow up on and document doctors' orders.
• Perform client assessments as needed.
• Occasional on call after hours and/or weekends.
