Job summary

Qualifications
- Current RN license valid in NY (or NJ per ad).
- Minimum of one year of clinical (medical/surgical) experience.
- Hospice/home health care experience preferred.
- Solid computer skills; familiarity with EMR is a plus.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Administer medications and provide wound care, treatments, and comfort care.
- Monitor clients' conditions and report changes to the Clinical/Client Services Manager.
- Follow up with, execute, and document doctors' orders accurately.
- Perform client assessments as necessary.
- Occasional on-call after hours and/or weekends.
