Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify the patient’s/family’s physical, psychosocial, and environmental needs, reassessing no less than every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family responses to hospice care.
• Collaborate with the patient/family, attending physician, and other members of the IDG in daily care.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient’s condition.
• Function in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.

About This Role

As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, continuously evaluating performance and collaborating with other professionals to implement treatment strategies.

Requirements

  • Current and unrestricted RN licensure in the state of practice
• Experience:
  • 1+ years of clinical experience preferred
• Licenses & Certifications:
  • Current RN certification in Oklahoma required
  • Current driver’s license and valid vehicle insurance required
  • Current CPR certification or ability to complete within 90 days of hire preferred
