Job summary

Wellstar Health System is seeking a Hospice RN Case Manager to provide comprehensive home hospice care across the West Georgia area. The role requires strong empathy, clinical judgment, and collaboration with physicians, social workers, chaplains, and a care team. Travel is involved within the service territory and mileage is reimbursed.

The position is fulltime (40 hours per week) with 8-hour shifts in the field and requires adaptability to varying schedules and patient needs.

Qualifications

• Associates degree or nursing diploma required.
• Minimum 2 years acute care or hospice/home care experience.
• Current BLS certification required.

Responsibilities

• Provides ongoing patient care according to the interdisciplinary care plan and maintains contact with patients and families.
• Educates patients and families on disease processes, symptom management, medications, and nutrition.
• Assesses symptoms and medication effectiveness; documents changes and communicates with the physician.
• Coordinates care with the interdisciplinary team and facilitates transfers when needed.
• Prepares patients and families for death and provides supportive care.
