Job summary

Visiting Nurse Health System is seeking a Full-time Registered Nurse Case Manager for Hospice, covering North Fulton and Dekalb County. The role coordinates comprehensive care plans, educates families, and ensures quality outcomes in a home-based setting.

You will collaborate with an interdisciplinary team, submit OASIS/documents timely, and supervise LPNs/HHAs as needed. GA licensure, CPR, and home health experience are preferred.

Qualifications

• Graduate from an approved nursing program.
• Current GA RN license and CPR certification.
• Experience in home health or hospice setting preferred.
• Knowledge of Medicare/Medicaid and hospice/home health regulations.

Responsibilities

• Develop and coordinate individualized care plans with physicians and interdisciplinary team.
• Update and modify care plans as patient conditions change.
• Educate patients and families on disease processes, self-management, and prevention of hospitalizations.
• Complete and submit OASIS and visit notes within VNHS timelines.
• Communicate patient status changes clearly with physicians and team members.
• Provide emotional support and connect patients/families with community resources as needed.
• Supervise LPNs and HHAs as per VNHS policy.
• Participate in SOC/ROC calls and weekly team conferences.
• Deliver high-quality home-based nursing care following VNHS standards.
• Maintain competencies through in-services and continuing education.
