Job summary

Wellstar Health System seeks a Full-time Day Hospice RN to travel daily within Wellstar territory, providing medical care and support to hospice patients and families. The role emphasizes empathy, clinical excellence, and coordination with an interdisciplinary team.

Two years of hospice or acute care nursing experience is preferred; Georgia RN licensure is required. Travel, strong communication, and adherence to DEA guidelines for medication management are essential.

Qualifications

• Associates/Bachelor Nursing preferred.
• RN license required in Georgia (or compact).
• 2+ years hospice/home health experience preferred.
• Excellent communication and computer skills.

Responsibilities

• Provides ongoing patient care according to the interdisciplinary care plan.
• Maintains ongoing contact with patients and families (phone calls, visits).
• Instructs on disease processes, symptom management, medications, and nutrition.
• Manages medications per orders; educate on safe storage and destruction per guidelines.
• Assesses symptoms, medication effectiveness, and reports to physician.
• Coordinate transfers to appropriate care facilities when needed.
• Maintains documentation in the electronic medical record.
• Participates in performance improvement activities and department committees.
