The Wellstar Health System is seeking a dedicated RN Case Manager for Home Hospice within Wellstar communities in Georgia. Full-time day shift; hospice care for terminally ill patients and their families with emphasis on empathy, communication, and coordinated care.

Requirements include an RN license in Georgia, BLS certification, and 2+ years in acute care or hospice/home care. Travel by personal vehicle is expected; mileage reimbursements provided.

Competenze

• Associates degree or diploma in nursing; bachelor’s degree preferred.
• RN licensure in Georgia; multi-state compact acceptable.
• BLS certification required; CHPN preferred.
• Valid Georgia driver’s license and private vehicle insurance.

Mansioni

• Provides ongoing patient care per interdisciplinary care plan.
• Maintains contact with patients/families via calls and visits.
• Educates on disease processes, symptom management, medications, and nutrition.
• Coordinates care with interdisciplinary team and external partners.
• Assesses symptoms and documents changes; facilitates transfers when needed.
• Maintains accurate EMR documentation and compliance with policies.
• Supports caregivers and prepares family for end-of-life needs.
