Job summary

Wellstar Health System in Georgia is seeking an in-field Home Hospice RN Case Manager to provide end-of-life care with empathy and clinical excellence across West Georgia Medical surrounding areas.

The role focuses on coordinating interdisciplinary care, educating families, and supporting patients through disease progression, with mileage reimbursement and autonomy in a hospice setting.

Qualifications

• Associates Degree or Nursing Diploma; Bachelors preferred
• BLS certification required at hire
• RN license (single state) or compact license
• Georgian driver's license; private vehicle insurance required
• Minimum 2 years acute care or hospice/home care experience
• Emotional maturity and interpersonal skills
• Strong assessment and clinical judgment
• Knowledge of age-related developmental care
• Proficient in EMR documentation and patient care records

Responsibilities

• Provides ongoing patient care per interdisciplinary care plan.
• Maintains contact with patients/families via calls or visits.
• Educates on disease processes, symptoms, medications, and nutrition.
• Manages medications, ensuring availability and safe storage; educates on disposal.
• Assesses symptoms and response to treatment; informs physician.
• Documents changes and plans to address disease progression.
• Coordinates transfers to appropriate care facilities when needed.
• Ensures patient safety and monitors for abuse or neglect.
• Supports care planning and communicates with the team.
• Participates in performance improvement activities.
