Job summary

Patriot Healthcare is seeking a dedicated Registered Nurse Case Manager to coordinate and direct hospice services based on physician orders and patient plans. You will apply the nursing process to provide comfort, dignity, and support at life’s important moments within an interdisciplinary team.

The role requires SC RN licensure, a nursing degree, and prior hospital or long-term care experience. Hospice experience is preferred, with a patient-centered approach and strong documentation skills.

Qualifications

• Nursing degree from an accredited college or university.
• Active RN license in South Carolina.
• Hospice experience preferred.
• Minimum 1 year of hospital/long-term care nursing experience preferred.

Responsibilities

• Steward the mission and philosophy of Patriot Healthcare, focusing on human dignity and compassionate care.
• Serve as a member of the Interdisciplinary Team (IDT) and attend meetings.
• Follow the Nursing Standards of Patriot Healthcare and applicable state/federal hospice regulations.
• Document nursing care accurately to support professional billing and regulatory compliance.
• Coordinate hospice nursing care in accordance with current rules and guidelines.
• Report to the Director of Clinical Services as needed regarding nursing operations.
• Participate in hospice quality assurance programs.
• Build relationships with contracted facilities to integrate hospice care into the facility plan of care.
• Supervise Hospice Aide care and CNA scheduling as applicable.
