Job summary

Patriot Healthcare in Greenville, SC is seeking an experienced RN Case Manager to coordinate and direct hospice services in collaboration with the Interdisciplinary Team. The RN will apply the nursing process to patient plans of care and ensure quality of life.

Must hold an active South Carolina RN license, and 1 year of nursing experience in hospital, long-term care, or related setting is preferred. Hospice experience is also preferred.

Qualifications

• Must hold an active South Carolina RN license.
• 1 year of nursing experience in hospital, long-term care facility, or related health care setting preferred.
• Hospice experience also preferred.

Responsibilities

• Coordinate and direct hospice services according to physician orders and patient plan of treatment.
• Serve as a member of the Interdisciplinary Team (IDT) and participate in meetings.
• Maintain nursing documentation and ensure compliance with hospice policies and regulations.
• Provide written and oral reports to the Director of Clinical Services as needed.
• Supervise Hospice Aide care and CNA schedules.
• Report occupational exposures per hospice policy.
