Job summary

Patriot Healthcare is seeking a dedicated RN Case Manager to coordinate and direct hospice services in accordance with physician orders and patient plans. As part of the interdisciplinary team, you will ensure compassionate care, documentation, and compliance with nursing standards.

The role requires SC RN licensure with a nursing degree, and hospice experience is preferred. We offer a supportive, mission-driven environment with comprehensive benefits and flexible scheduling.

Qualifications

• Nursing degree from an accredited college or university is required.
• RN license in the State of South Carolina is required.
• Hospice experience is preferred.
• 1 year of nursing experience in hospital, long-term care, or related facility is preferred.

Responsibilities

• Coordinate and direct hospice services according to physician orders.
• Serve as a member of the Interdisciplinary Team (IDT) and attend meetings.
• Follow hospice policies, state and federal regulations for nursing services.
• Document direct patient care for accurate billing and compliance.
• Supervise Hospice Aide and CNA schedules as needed.
• Report occupational exposures per hospice policies.
