Job summary

Patriot Healthcare is seeking an RN Case Manager to coordinate and deliver hospice services with compassion and clinical excellence in Greenville, SC. You will work within an interdisciplinary team to provide dignified end-of-life care, ensuring adherence to physician orders, and patient plans of care.

The role emphasizes nursing standards, thorough documentation, and collaboration with physicians, facilities, and families to optimize comfort and quality of life.

Qualifications

• Must possess a Nursing Degree from an accredited college or university and be licensed as a Registered Nurse in the State of South Carolina.
• 1 year of experience as a nurse in a hospital, long-term care facility, or other related health care facility is preferred.
• Hospice experience also preferred.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plan of treatment.
• Attend Interdisciplinary Team (IDT) meetings and support the care team.
• Follow Hospice policies, nursing standards, and regulatory guidelines.
• Document direct patient care clearly to support billing and quality assurance.
• Supervise Hospice Aide care and CNA schedules as needed.
• Communicate with hospice physician, attending physician, and facility staff to coordinate care.
