Job summary

Patriot Healthcare is seeking an experienced RN Case Manager in South Carolina to coordinate hospice services and lead an interdisciplinary team. The role emphasizes compassionate patient care, regulatory compliance, and effective communication with patients, families, and facility staff.

The ideal candidate holds a SC RN license, has a Nursing degree, and brings 1 year of hospital/long-term care experience; hospice exposure is a plus.

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Licensed as a Registered Nurse in the State of South Carolina.
• Hospice experience preferred; 1 year of hospital/long-term care experience preferred.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plan.
• Serve as a member of the Interdisciplinary Team (IDT) and attend meetings.
• Document and report to the Director of Clinical Services; participate in quality assurance.
• Supervise Hospice Aide care and CNA schedules; ensure compliant care delivery.
