Job summary

Concordia Lutheran Ministries in Valencia, PA is seeking hospice Registered Nurse Case Managers to support patients in their homes and inpatient locations across Beaver County, Moon Township, Monroeville, Harmar, Cheswick, and surrounding areas.

Full and part‑time positions are available with weekend coverage once a month and on‑call rotation, offering competitive pay, benefits, and opportunities for growth within Good Samaritan Hospice’s interdisciplinary team.

Qualifications

• Current RN license in the state of Pennsylvania.
• Demonstrates excellent clinical skills.
• Solid computer and documentation skills.
• Excellent organizational skills with attention to detail.
• Outstanding communication skills (both written and verbal).

Responsibilities

• Assess and monitor the physical, emotional, and spiritual needs of our hospice patients and create individualized care plans.
• Educate patients and their families about the disease process, care options, and end-of-life decisions.
• Provide pain management and symptom control measures to alleviate discomfort and improve quality of life.
• Advocate for our patients, ensuring their wishes and preferences are respected and honored.
