Job summary
Vitality Hospice & Palliative Care is seeking a Registered Nurse Case Manager (RNCM) in Pennsylvania to join our compassionate team. This full-time role covers admissions, care planning, and coordination with the interdisciplinary team to deliver high-quality hospice care.
You will travel within the service area, maintain timely documentation, and educate patients and families. Licensed PA RN with hospice or home health experience preferred; schedule is Monday–Friday, 8:00 AM–5:00 PM with
Qualifications
- Strong communication, organizational, and interpersonal skills are required.
- Home health or hospice nursing experience is preferred.
- Valid driver’s license and ability to travel within the service area are required.
Responsibilities
- Conduct patient admissions and ongoing nursing assessments.
- Develop and implement individualized plans of care based on patient needs and goals.
- Collaborate with hospice physicians and the interdisciplinary team (IDT).
- Supervise and coordinate care with home health aides, social workers, and chaplains.
- Educate patients and caregivers about disease progression, symptom management, and available resources.
