Job summary

Serenity Hospice Care Pennsylvania is seeking a compassionate Registered Nurse (RN) Hospice Case Manager in Allentown, PA. This full-time role offers no nights, no weekends, mileage reimbursement, and comprehensive benefits while focusing on patient- and family-centered end-of-life care.

You will perform home visits, assess patients, develop care plans, and work with a multidisciplinary team to support comfort, dignity, and quality of life for patients and families in the Allentown area.

Qualifications

• Current Pennsylvania RN license is required.
• Minimum 1 year of nursing experience, hospice/home health/case management preferred.
• Valid driver’s license with reliable transportation.
• Strong clinical, organizational, and communication skills.

Responsibilities

• Manage and coordinate care for an assigned hospice caseload.
• Perform comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing visits in patients’ homes and other care settings.
• Collaborate with physicians, social workers, chaplains, aides, and other team members.
• Educate patients and families on disease progression, symptom management, and medications.
• Monitor patient status and adjust care plans as needed.
• Ensure accurate, timely clinical documentation in accordance with hospice regulations.
• Participate in interdisciplinary team meetings and care planning.
• Serve as a patient and family advocate throughout the hospice journey.
