Job summary

Patriot Healthcare is seeking an experienced RN Case Manager in South Carolina to coordinate and deliver hospice services with a compassionate, patient-centered approach. You will work with interdisciplinary teams to ensure dignity and comfort for patients and families, while maintaining compliant nursing practice and documentation.

The position requires an SC RN license and prior hospice or long-term care experience is preferred. Full-time position with competitive pay and benefits is offered.

Qualifications

• Must be a licensed RN in South Carolina.
• Experience in hospice/long-term care preferred.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plan of treatment.
• Serve on the Interdisciplinary Team (IDT) and attend meetings.
• Document nursing care and provide timely reports to Clinical Services leadership.
• Supervise Hospice Aide care and CNA schedules as needed.
• Maintain up-to-date patient records and coordinate with physicians and facilities for seamless care.
