Job summary

Life Care Home Health is seeking an experienced Registered Nurse Case Manager to coordinate admissions and nursing care for patients in their homes. You will work with an Interdisciplinary Group/Team and families to plan and deliver comprehensive care, including on-call support and travel to patient homes in the region.

The role requires hospice experience, current RN licensure, and a valid driver’s license, with a focus on quality care, regulatory compliance, and compassionate service.

Qualifications

Two (2) years nursing experience in a clinical care setting and one (1) year full-time hospice experience.
Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.

Responsibilities

Assess hospice patients using Nursing Assessment and LCD criteria to identify needs.
Develop and document the plan of care.
Provide nursing care with infection control measures.
Coordinate with Interdisciplinary Group/Team, patient/family, and referring agency.
Communicate with attending physicians and hospice physicians involved in care.
Submit all required paperwork in a timely manner.
Participate in hospice-sponsored in-service programs and QAPI activities.
Meet productivity standards and provide on-call services.
Ensure effective communication with patients, families, and health care professionals.
