Job summary

boost-home-health-careers is seeking a compassionate Registered Nurse to join our Home Health Case Management team. You will manage a caseload, perform comprehensive assessments, and coordinate services with an interdisciplinary team to ensure high-quality patient home health care.

Qualifications

RN license with active state license.
At least 1 year of experience in home health or case management.
Strong critical thinking and communication abilities.
Comfort using EHR systems (Kantime, Axxess, HCHB, WellSky preferred).
Reliable transportation and a valid driver’s license.

Responsibilities

Manage the Care Journey: serve as the primary contact for a caseload of home health patients, coordinating services.
Conduct Comprehensive Assessments: complete OASIS assessments and develop care plans with the interdisciplinary team.
Coordinate Services: work with physicians, nurses, therapists, social workers, and home health aides for timely care.
Monitor and Evaluate: track patient progress and update care plans; communicate with families/caregivers.
Documentation: maintain accurate and timely clinical documentation per Medicare guidelines and agency policy.
Advocate for the Patient: provide education, resources, and emotional support to promote independence and wellness.
