Job summary

Life Care Home Health in Henderson, NV is seeking a Full-Time RN Case Manager to admit and coordinate nursing care for patients in their residence. You will collaborate with the Interdisciplinary Group/Team, patients, families, and referring agencies to ensure high-quality, coordinated care.

You will assess hospice patients, document care plans, provide nursing care with infection control, and participate in in-service programs and QAPI.

Qualifications

Active RN license and eligibility for state practice.
Experience with hospice or home health preferred.
Ability to coordinate care across disciplines and settings.

Responsibilities

Assess hospice patients to identify physical, psycho-social, and environmental needs.
Develop and document the plan of care in correlation with the assessment.
Provide nursing care utilizing infection control measures (OSHA).
Work with Interdisciplinary Team to meet patient/family needs.
Communicate with attending physicians and other care providers.
Submit required paperwork in a timely manner.
Participate in hospice-sponsored in-service programs and QAPI activities.
Promote hospice philosophy and administrative policies.
Meet productivity standards.
Perform on-call responsibilities and provide on-call services.
Communicate effectively with patients, families, team, and other health care professionals.
