Job summary

Infinity Hospice Care LLC is seeking a dedicated RN Case Manager to perform admissions and provide nursing care for patients in their homes. You will coordinate with the interdisciplinary team, patient and family, and referring agencies to ensure seamless care delivery.

Applicants must hold an active RN license and a valid driver's license, with hospice experience preferred. The role includes on-call responsibilities and travel to patient homes within service areas.

Qualifications

• Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
• Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Licensure: Current State license as a Registered Nurse; Current driver's license.
• Knowledge of state and federal regulations; Good interpersonal skills.

Responsibilities

• Perform admissions and administer nursing care for patients in their homes.
• Coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency.
• Develop and document the plan of care in correlation with the Assessment.
• Provide nursing care with infection control measures.
• Communicate with attending physicians and hospice physicians.
