Job summary

Lifecare-Home-Health-Family-1 is seeking a full-time RN Case Manager to perform admissions and coordinate nursing care for patients in their homes, working with the interdisciplinary team and families to ensure seamless hospice service.

Two years of RN clinical care experience with hospice, Nevada RN license and driver’s license, and excellent interpersonal skills are required. The position offers competitive pay, benefits, and opportunities for professional growth.

Qualifications

• Two years as a Registered Nurse in clinical care, plus one year in hospice.
• Graduate of an accredited diploma, ADN, or BSN program.
• Current State RN license and valid driver's license.
• Good interpersonal and communication skills.

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria to identify needs.
• Develop and document the plan of care with the assessment.
• Provide nursing care with infection control.
• Coordinate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending and hospice physicians involved in care.
• Submit all required paperwork on time.
• Participate in Hospice's QAPI program and ongoing competency.
• Meet productivity standards.
• Provide on-call services to patients and families as assigned.
• Communicate effectively with patients, families, and healthcare professionals.
• Travel to patient homes and office/support centers as needed.
