Job summary

Life Care Home Health is seeking an experienced Registered Nurse Case Manager to join our in-home care team in Nevada. You will perform admissions, administer nursing care, coordinating with the Interdisciplinary Group and families to plan and deliver personalized care.

The role emphasizes patient-centered coordination, infection control, timely documentation, on-call responsibilities, and travel to patient homes.

Qualifications

• RN with an active license in the relevant state.
• Two years of nursing experience in a clinical setting and at least one year of full-time hospice experience.
• Graduation from an accredited nursing school (Diploma/Associate/Bachelor).

Responsibilities

• Perform admissions and administer nursing care for patients in their homes.
• Coordinate care with the Interdisciplinary Group/Team and families.
• Develop and document the plan of care and ensure timely paperwork.
• Provide on-call nursing services and travel to patient homes as needed.
• Maintain infection control practices and comply with regulations.
