Job summary

Infinity Hospice Care, part of the LCHH family, seeks an RN Case Manager to provide admissions and nursing care to patients in their homes across Arizona. You will coordinate care with the Interdisciplinary Group, patients, families, and referring agencies to ensure a seamless plan of care.

Responsibilities include assessing hospice patients, documenting care plans, delivering nursing care with proper infection control, and coordinating with physicians.

Qualifications

• Two years RN clinical experience in a care setting.
• One year full-time hospice experience preferred.
• Graduate from an accredited nursing program.
• Current State RN license and valid driver’s license.

Responsibilities

• Assess hospice patients and identify physical, psychosocial, and environmental needs.
• Develop and document the plan of care in coordination with the Interdisciplinary Group.
• Provide nursing care with proper infection control measures.
• Coordinate with physicians, hospice team, and families to ensure care needs are met.
