Job summary

Life Care Home Health Family in Las Vegas, NV seeks an experienced RN Case Manager to support admissions and provide in-home nursing care for patients across the care continuum. You will coordinate with the Interdisciplinary Group/Team, patients, families, and referring agencies to plan and deliver personalized care.

The role requires a valid RN license, at least two years in clinical nursing and one year in hospice, a graduate nursing degree, and ability to travel.

Qualifications

Two years of experience as a Registered Nurse in a clinical care setting
One year full-time hospice experience
Graduate of an accredited Diploma, Associate or Baccalaureate Nursing program
Current State RN license and valid driver's license

Responsibilities

Perform admissions and administer nursing care in patients' homes
Coordinate care with Interdisciplinary Group/Team, patient/family and referring agency
Communicate with attending physicians and hospice physicians involved in patient care
Submit required paperwork in a timely manner
Provide on-call nursing services as assigned
Maintain infection control measures and quality standards
