Job summary

Infinity Hospice Care Reno is seeking an experienced RN Case Manager to perform admissions and provide nursing care for patients in their homes across Nevada. You will coordinate with the interdisciplinary team, develop the plan of care, and ensure timely paperwork and communication with physicians involved in patient care.

The role requires a valid RN license, at least 2 years of clinical RN experience and 1 year in hospice, plus a driver’s license.

Qualifications

• Two years experience as a Registered Nurse in a clinical care setting
• One year full-time hospice experience
• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
• Current State RN license and valid driver’s license

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria to identify physical, psycho-social, and environmental needs
• Develop and document the plan of care in correlation with the assessment
• Provide nursing care utilizing infection control measures
• Coordinate with Interdisciplinary Group/Team to meet patient/family needs
• Communicate with attending hospice physicians and other physicians involved in the patient’s care
• Timely submission of all required paperwork
• Participate in Hospice-sponsored in-service programs and the Hospice’s QAPI program
• Promote Hospice philosophy and administrative policies
• Meet productivity standards
• Performs on-call responsibilities and provides on-call services
• Provide effective communication to patients, families, team members, and health professionals
• Transportation: Reliable transportation and valid auto liability insurance
