Job summary

Interim HealthCare is seeking a Hospice RN Case Manager to serve Charleston, Berkeley, and Dorchester Counties. You will assess patients, develop IDG plans, coordinate care with providers, and support families with compassionate end-of-life care.

The role emphasizes on-call duties, ongoing assessments, and supervising LPN/LVN, Hospice Aide, and Homemaker. Graduate of an accredited nursing program with SC RN license, CPR certification, and hospice experience are required; driver’s license and

Qualifications

• Graduate of an accredited school of nursing.
• Active RN license in South Carolina.
• 12 months RN experience within last 5 years (or evidence of current clinical knowledge).
• For new graduates: 1200 verified hours or completion of a approved Preceptor Program.
• 1 year of hospice care experience.
• 1 year of precepting or training preferred.
• CPR certification.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Maintains a specified case management load.
• Completes initial/comprehensive patient/family assessment to determine hospice needs.
• Participates in on-call duties.
• Performs ongoing assessments of patient’s physical and psychosocial status.
• Develops a care plan in collaboration with IDG, patient/family, and physician.
• Revisions to IDG plan as needs change.
• Coordinates care among IDG members and external providers.
• Evaluates and coordinates services per the IDG plan.
• Directs services and supervises LPN/LVN, Hospice Aide, and Homemaker.
• Facilitates consideration of higher level of care when indicated.
