Job summary

Interim HealthCare in Charleston, SC seeks a 13-week Hospice RN Case Manager for a contract role providing compassionate hospice care and coordinating individualized plans with the IDG. You will manage patient loads, assess needs, and ensure high-quality end-of-life care.

The role requires an active SC RN license, CPR, and at least one year in hospice care. Independent workstyle and strong EMR skills are essential for success in this position.

Qualifications

• Graduate of an accredited school of nursing.
• Current, active RN license in good standing in South Carolina.
• At least one year of hospice-care experience.
• Must be able to work independently with limited orientation.
• Strong clinical documentation skills with attention to detail.
• Tech-savvy and comfortable with EMR systems.
• One year of precepting or training is preferred.
• Current CPR certification.

Responsibilities

• Maintain an assigned hospice case-management load.
• Complete initial and comprehensive patient and family assessments.
• Assess illnesses and perform physical assessments with emphasis on pain and symptoms.
• Conduct ongoing assessments of physical and psychosocial needs.
• Develop individualized care plans with the IDG, patient, family, and physician.
• Revise the plan of care as needs change.
• Coordinate care among IDG members and external providers.
• Evaluate and coordinate services per the plan of care.
• Direct and supervise services provided by LPNs/LVNs, Hospice Aides, and Homemakers.
• Facilitate transitions to other levels of care when appropriate.
• Participate in on-call duties as defined by policy.
• Complete timely clinical documentation.
