Job summary

Interim HealthCare is seeking a compassionate Hospice Registered Nurse (RN) to lead with empathy and clinical excellence in Greenville, SC. This float role covers Upstate and Columbia service lines, delivering patient-centered end-of-life care and coordinating IDG plans with families and physicians.

Requirements include an accredited RN program graduate, SC RN or Compact license, and at least one year of nursing experience; hospice experience is preferred.

Qualifications

• Graduate of an accredited RN program.
• Active South Carolina RN license or Compact License.
• Minimum one year of nursing experience.
• Hospice experience preferred.

Responsibilities

• Completes initial/comprehensive assessment of the patient/family to determine hospice needs.
• Completes ongoing assessments of the patient’s physical and psycho-social status.
• Initiates and develops a plan of care with IDG, patient/family, and physician.
• Revise the IDG plan of care as patient/family needs change.
• Coordinate care among IDG members and external providers to ensure continuous assessment and implementation of the plan.
• Evaluate and coordinate services for the patient/family per the IDG plan.
• Administer medications and treatments as prescribed.
• Provide education and interventions for pain and symptom management.
