Job summary

Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our nursing team delivering compassionate end-of-life care in the Tri-County area of Berkeley, Charleston and Dorchester, SC.

You will coordinate services, provide hands-on nursing care, and guide patients and families through the hospice journey, backed by a supportive team and a comprehensive benefits package.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical surgical/acute care setting; hospice experience a plus.
• Proficient with EMR systems and CPR certification if required by state.

Responsibilities

• Empower Through Engagement: respond to clinical referral information with care and expertise.
• Collaborative Care Champion: assess eligibility and suitability for hospice services with Clinical Director.
• Nurturing Direct Care: deliver hands-on hospice nursing care and coordinate services.
• Nursing with Heart: apply nursing skills to ease pain and provide comfort.
• Educator and Advocate: educate patients and families to create personalized care plans.
• Call of Compassion: participate in call duties to be there when it matters most.
