Job summary

Ennoble Care is seeking a Hospice Registered Nurse Case Manager to serve the Arlington County region in Virginia. You will work within an interdisciplinary team to provide skilled nursing assessments, planning, and care to maximize comfort for patients and support for families.

The role requires an active VA RN license and at least two years of nursing experience, preferably in hospice. You will coordinate care with physicians, hospice staff, and external agencies, while maintaining accurate EMR

Qualifications

• Current VA RN license required.
• At least two years of professional nursing experience, hospice experience preferred.
• Ability to assess and respond to needs of patients and families across settings.
• Strong documentation and interdisciplinary collaboration skills.

Responsibilities

• Manage a patient caseload including assessment, planning, implementation and evaluation.
• Coordinate with attending physicians, Hospice staff, and other agencies for optimal care.
• Maintain regular communication with physicians and families.
• Document accurately in electronic medical records.
• Participate in Interdisciplinary Group meetings and care conferences.
• Assist with education of caregivers and coordination of resources.
• Provide support at time of death and during bereavement as needed.
• Adhere to agency policies and state/federal regulations.
