Job summary

Jefferson Health at Home by BAYADA Hospice is seeking a compassionate Registered Nurse (RN) Case Manager to provide palliative care at home in Montgomery and Bucks Counties, PA. As a member of a multi-disciplinary team, you will visit patients in their homes and coordinate care with clinicians.

You will perform medication administration, wound care, treatments, monitoring, and documentation, with on‑call duties after hours and weekends.

Qualifications

• Active and current RN license in PA.
• Hospice experience preferred; not required.
• Ability to travel to designated cases.

Responsibilities

• Making home visits to clients in designated geographic territories.
• Administering medications, wound care, treatments, and comfort care.
• Monitoring clients’ conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Occasional on call after hours and/or weekends
