BrightSpring Health Services seeks a compassionate Registered Nurse Case Manager for Hospice in Yazoo City, MS. Join a team delivering patient-centered care at a critical life stage, with support and opportunities for professional growth.

As a RN Case Manager, you will assess needs, develop care plans, provide direct nursing care, manage pain and symptoms, and educate families. On-call rotation and independent work with team support are included.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch
