Job summary

Adoration Health is seeking a registered nurse case manager to join our hospice team in Yazoo City, MS. The RN will deliver compassionate, patient-centered care and coordinate plans with families and the interdisciplinary team.

Qualifications

• Graduate of an accredited nursing program with current licensure in the state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create and manage hospice care plans aligned with patient wishes and goals.
• Direct nursing care including medications, treatments, and interventions.
• Provide pain and symptom management and education to families.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by the local branch.
